Reflections On A Brother’s Cancer Journey

This month, I reflect upon a poignant cancer journey – the passing of my brother. I delve into the lessons learned, the emotions felt, and the profound impact of his story on our lives.

A Sudden Diagnosis

My brother died at the age of 53, in the USA, less than two months after being diagnosed with stage 4 lung cancer. His death resulted from a drug-induced inflammation of his lungs, leading to respiratory failure.

My brother, a non-smoker who led an active lifestyle and cherished the outdoors, suddenly faced a health crisis when he developed dizziness and double vision just two weeks before his diagnosis. Subsequent investigations uncovered multiple metastases in his brain, causing alarming pressure symptoms. The primary was a large non-small cell lung cancer.

A Glimmer of Hope

To address the swelling in his brain, he began high-dose steroid medication, which provided temporary relief until he underwent surgery to remove the largest brain lesion. Remarkably, he emerged from the surgery without complications and experienced a rapid recovery.

Amidst our family’s apprehension, a glimmer of hope emerged with the discovery that genetic profiling of his lung cancer revealed a treatable mutation. This revelation opened the door to targeted immunotherapy, offering a significantly improved prognosis of 5–10 years or more.

A Bucket List and Dreams of Adventure

My brother, ever the optimist, compiled a bucket list brimming with his dreams and aspirations. Though he cherished the home he had lovingly built for his family a decade ago, he felt a desire for improvements and upgrades. Immersing himself in various home improvement projects, he poured his heart into enhancing their living space.

An avid traveler, he found solace and joy in exploring new destinations. Hawaii held a special place in his heart, serving as his ultimate happy place. To commemorate his 25th wedding anniversary with the wife he adored, he meticulously planned a spectacular family holiday to Hawaii in March.

A Setback During the Holidays

Two weeks post-surgery, as Christmas approached, my brother experienced a resurgence of double vision. Our hearts sank when subsequent brain imaging unveiled substantial growth of existing metastases alongside multiple new lesions. To combat the swelling, he resumed a regimen of high-dose steroids.

In tandem with medical interventions, my brother embraced a ketogenic eating plan under my guidance. Within days of adopting this new diet, he expressed feeling a surge in energy and noticed gradual weight loss. Determined to support his journey towards improved metabolic health, I flew over on Boxing Day to provide assistance, introduce him to fasting, and offer guidance every step of the way.

Alarming Changes in Behavior

Observing his erratic sleep patterns, I couldn’t help but notice that my brother was only managing 2–3 hours of rest each night. Despite his lack of sleep, he appeared to possess boundless energy, tirelessly rearranging the house from dawn till dusk. A steady stream of parcels from Amazon arrived throughout the day, containing expensive bikes, watches, cameras, home appliances, and computers.

However, alongside his frenetic activity, signs of distress began to surface. He grew increasingly irritable when questioned about his spending habits, displaying an unusual emotional fragility and tearing up when discussing certain aspects of his life.

Despite his apprehensions regarding potential side effects, my brother began targeted therapy for his lung cancer at the end of December. Remarkably, he suffered no side effects.

Recognizing Acute Mania

Within a matter of days, I began to recognize alarming signs of acute mania in my brother—a state characterized by heightened arousal and erratic behavior. What troubled me most was that he had no prior history of mental health issues.

Attributing his symptoms to the high-dose steroids prescribed to alleviate pressure symptoms in his brain, I grew increasingly concerned. Despite my attempts to broach the subject of drug-induced hyper-arousal, my brother remained dismissive, insisting that he felt invincible.

Challenges in Coordinating Care

Navigating this challenging situation proved to be an uphill battle. Attempts to involve his oncology team in reviewing his steroid dosage were difficult, as it became apparent that their focus was solely on oncological matters.

Amid our efforts to navigate the complexities of his condition, my brother encountered another hurdle—he developed acute urinary retention, a distressing condition marked by the sudden inability to pass urine with a significant residual volume in the bladder. This necessitated his admission to the hospital for urgent management.

Seizing this opportunity, I communicated with his treating team, apprising them of his concerning mental health symptoms. Yet, once again, securing a psychiatric assessment proved to be a formidable challenge, underscoring the systemic barriers hindering comprehensive care for patients facing multifaceted health issues.

Finding Hope Amid the Turmoil

After persistent advocacy, his steroid dosage was gradually tapered down, and he was initiated on medication to promote restful sleep. To our immense relief, his manic symptoms began to subside over the following week. With a renewed sense of hope, I departed for home after three weeks, reassured that my brother was receiving the appropriate care he needed.

During his hospitalization, he commenced radiation therapy for his brain, a crucial intervention aimed at addressing all his brain metastases. The treatment plan comprised a series of six radiation sessions.

Pneumonia and a Difficult Realization

Despite our efforts to optimize his health through dietary interventions, the treating doctors discouraged the continuation of the keto diet and fasting during his hospital stay.

Unfortunately, a few days later, he began experiencing labored breathing and required supplemental oxygen. Subsequent evaluation revealed pneumonia, prompting immediate treatment. However, despite medical intervention, a week passed with no signs of improvement, leaving my family and me grappling with unanswered questions about his deteriorating condition.

It was a sobering realization that approximately 2% of individuals receiving the immunotherapy treatment for lung cancer, like my brother, could develop a severe inflammatory lung condition known as interstitial lung disease. With a mortality rate as high as 50%, the gravity of the situation weighed heavily on us.

Saying Goodbye

In an attempt to manage the condition, high-dose steroids were administered. However, despite these efforts, his breathing continued to deteriorate. Eventually, he became so debilitated and exhausted that he required admission to the intensive care unit (ICU) for mechanical ventilation support.

My parents, sister, and I rushed to his side just in time to be there with him, ensuring he knew he was surrounded by the love of his family. In the end, he passed away peacefully, enveloped by the warmth of those who cherished him most.

Reflections on His Cancer Journey

Throughout my brother’s journey, he received exceptional care marked by remarkable compassion and dedication from every individual he encountered. Despite this, the ‘Standard Of Care’ within the US medical system can sometimes limit clinicians’ autonomy in decision-making. Additionally, the independent workings of individual specialties often hinder the provision of comprehensive care for patients with complex needs.

Honoring His Legacy

As I reflect on my brother’s cancer journey, I can’t help but feel saddened by the realization that the medication and treatments available for cancer management can sometimes pose greater risks than the disease itself.

Ultimately, my brother was surrounded by love and received the utmost care within the limitations of the US health system. Despite the hurdles he faced, his resilient spirit lives on in our hearts and memories.

May his journey through samsara be brief, and may he find ultimate peace and bliss in nibbana.
